Forum Abstract

The Center for Connected Health (CCH) is committed to transforming health care delivery to enable care where the patient is, when the patient needs it and to encourage preventative care and self care.  To achieve this vision, innovation is required since it turns our current care model upside down.  In our 15 year experience, we’ve seen innovation happen in four phases:  idea generation, proof of concept, reality-based demonstration and bringing a program to scale.  Four examples of this journey will be discussed including two that have been successful and two that were not.  Our SmartBeat (hypertension self-monitoring) program was trialed with a local employer and based on the findings of the trial, the platform was licensed to a start up that will bring it to the market. Likewise, when we implemented an automated coaching application to encourage overweight individuals to be more active, those in the program were twice as active as control subjects. This too will be part of the new company’s offerings.  On the other hand, we showed promising results when we enabled the relaxation response to be taught in the virtual world Second Life, but we were unable to move this beyond an early demonstration.  We also demonstrated that online follow-up visits for dermatologic care resulted in quality of care that was equal to face to face visits, but were unable to affect adoption of this strategy beyond the clinical trial.
